Support Worker

About the Role
We are seeking a highly motivated and enthusiastic Support Worker to join our team at Kent and Medway NHS and Social Care Partnership Trust. As a Support Worker, you will play a vital role in providing support to our service users, promoting their equality, dignity, and mental well-being.
Key Responsibilities
- Assess and plan alongside the needs of the service user, liaising with our Clinical Pathway Leads following Dialog+
- Deliver group-based interventions and influence and drive change
- Contribute to assessing clients referred to Mental Health Together or Mental Health Together Plus
- Work with individuals to facilitate access and engagement in a range of leisure and other community-based activities
- Assist in delivering a high standard of support to service users and their carers

Requirements
- Previous experience in healthcare
- NVQ Level 2 in Care/Community Mental Health Care or agreed equivalent experience
- Commitment to undertake and gain appropriate Level 3 qualification within an agreed time limit
