We are seeking a highly motivated and compassionate Support Worker to join our team at Kent and Medway NHS and Social Care Partnership Trust. As a Support Worker, you will play a vital role in providing high-quality care and support to our service users, promoting their equality, dignity, and mental wellbeing.
Key Responsibilities- Assess and plan alongside the needs of service users, liaising with Clinical Pathway Leads as necessary.
- Deliver group-based interventions and promote health and best practice, patient safety, and experience.
- Contribute to assessing clients referred to Mental Health Together or Mental Health Together Plus.
- Work with individuals to facilitate access and engagement in leisure and community-based activities, working across care boundaries.
- Assist in delivering a high standard of support to service users and their carers, promoting their equality, dignity, and mental wellbeing.
- Previous experience in healthcare, with a commitment to undertaking and gaining a Level 3 qualification within an agreed time limit.
- NVQ Level 2 in Care or Community Mental Health Care, or agreed equivalent experience.
