About the Role

We are seeking a highly skilled and experienced Clinical Triage Practitioner to join our team at Queen Elizabeth's Foundation for Disabled People. As a key member of our multidisciplinary team, you will be responsible for assessing and triaging referrals, making clinical decisions, and developing rehabilitation plans for individuals with neurological impairments.

Key Responsibilities
  • Conduct comprehensive assessments of clients with complex neurological presentations, utilizing clinical reasoning skills and evidence-based practice.
  • Develop and implement rehabilitation plans, taking into account individual needs and goals.
  • Collaborate with multidisciplinary teams, including healthcare professionals, therapists, and carers, to ensure seamless care coordination.
  • Communicate effectively with clients, families, and healthcare professionals, using tact and persuasive skills to facilitate smooth transitions and care pathways.
  • Stay up-to-date with best practices, research, and guidelines in neurological rehabilitation, applying this knowledge to inform clinical decision-making.
Requirements
  • Registered nurse or postgraduate physiotherapist/occupational therapist with current NMC or HCPC registration.
  • Minimum 5 years of post-qualification clinical experience, with a focus on neurological rehabilitation.
  • Proven ability to work autonomously and as part of a multidisciplinary team.
  • Excellent communication and interpersonal skills, with the ability to work with clients, families, and healthcare professionals.
  • Strong analytical and problem-solving skills, with the ability to make informed clinical decisions.
