Job Description:

We are seeking a highly skilled Clinical Triage and Rehab Support Specialist to join our team. The successful candidate will be responsible for providing clinical triage and rehabilitation support to patients, working closely with our multidisciplinary team to ensure the best possible outcomes.

Key Responsibilities:
  1. To be professionally and legally accountable for all aspects of your own and delegated work.
  2. To work within codes of practice and professional guidelines.
  3. To undertake specialist holistic rehab assessment, making use of clinical reasoning skills.
  4. To control and manage triage calls effectively.
  5. To evaluate patient progress and/or change, reassess and understand the clinical rationale for altering a rehabilitation plan and interventions.
  6. To understand and be acutely aware of significant medical observations during assessment and treatment of patients.
  7. To complete a written and thorough assessment of all clients requiring an in-person assessment.
  8. To work with the business development team, and admissions and discharge coordinator, sharing findings of your assessment in a proactive and timely manner.
  9. To maintain an effective and efficient communication line with HOCS.
  10. To maintain up-to-date clinical skills and rotate into clinical team as appropriate.
  11. To be responsible for the handover process to the relevant QEF CRC clinical staff.
  12. To demonstrate the importance of gaining patient and where appropriate family, carer or additional professional consent for all interactions and assessments.
  13. To effectively communicate with patients, family members, carers and colleagues.
  14. To work in line with QEF policies and procedures.
  15. To work within your own professional body, demonstrating a good working knowledge of national and local standards of practice.
  16. To organise and manage own time in keeping with line manager expectations.
  17. To conduct and contribute to risk assessments and any relevant health and safety assessments.
  18. To be responsible for the handover process to the relevant QEF CRC staff of resulting recommendations from risk and health and safety assessments.
  19. To take an active part in the development of the QEF CRC service and in supporting the organisational aims and objectives.
  20. To work effectively within the service and liaise appropriately with members of the MDT.
  21. To ensure that accurate and timely written records are kept.
  22. To participate in the ongoing development of the role and support the development of the protocols and processes in the Triage.
  23. To support the HOCS, General Manager and therapy team leads with day-to-day planning and delivery of a high-quality service.
  24. To participate in annual audit plans for the Service and annual staff surveys.
  25. To undertake evidence-based practice to further own clinical practice and maintain competency.
Requirements:
  1. 5 years + post qualification clinical experience.
  2. 3 years + Community experience.
  3. 3 years + Experience with neurologically impaired adults.
  4. Solid experience of working in a team/MDT environment.
  5. Experience of working in a rehab setting.
  6. Experience of working autonomously.
  7. Experience of carrying a caseload.
  8. Experience of making decisions based on clinical rationale.
  9. Registered nurse or post graduate physiotherapist / occupational therapist.
  10. Current NMC or HCPC registration.
  11. Completed Mentorship and/or clinical supervision training.
  12. Evidence of ongoing professional development.
