About the Role

We are seeking a highly skilled and experienced Clinical Triage Practitioner to join our team at Queen Elizabeth's Foundation for Disabled People. As a key member of our rehabilitation team, you will be responsible for assessing clients with a range of clinical presentations, including neurological diagnoses, and making recommendations on their clinical pathway.

Main Responsibilities

  • To be professionally and legally accountable for all aspects of your own and delegated work.
  • To work within codes of practice and professional guidelines.
  • To undertake specialist holistic rehab assessments, including those with complex presentations and multi-pathologies, making use of clinical reasoning skills.
  • To control and manage triage calls effectively, using skilled questioning, active listening, and excellent verbal communication skills.
  • To evaluate patient progress, write reports, and be responsible for the handover process.
  • To effectively communicate with patients, family members, carers, and colleagues, using tact and persuasive skills.
  • To work in line with QEF policies and procedures.

Our Care and Rehabilitation Centre is a state-of-the-art 48-bedded rehab centre, offering four different client pathways, including Neuro Rehabilitation, General Rehabilitation, Complex Neurological Care, and Outpatient/Respite Services.

Person Specification

  • Ability to demonstrate and evidence effective clinical skills equivalent to Band 6 competency level.
  • Able to demonstrate sound theoretical knowledge of varied neurological conditions.
  • Able to complete a full and thorough neurorehab assessment.
  • Familiar with and understands requirements for the use of varied pieces of rehabilitation equipment.
  • Understanding of what rehabilitation potential and change means.
  • Experience of setting goals and prioritizing workload.
  • Well-developed written and verbal communication skills.
  • Able to communicate effectively with clients, carers, and team members.
  • Ability to triage a diverse range of referrals and make decisions.
  • Able to identify risks and act accordingly.
