Clinical Triage and Rehab Support Practitioner

Location: Leatherhead

Job Type: Permanent - Part-Time - 4 days a week

Duties
  1. Exercise professional and legal accountability for all aspects of your work.
  2. Work within codes of practice and professional guidelines.
  3. Conduct specialist holistic rehab assessments, including those with complex presentations and multi-pathologies, utilizing clinical reasoning skills.
  4. Effectively manage triage calls by using skilled questioning, active listening, and excellent verbal communication skills in a calm and professional manner.
  5. Evaluate patient progress and/or change, reassess, and understand the clinical rationale for altering a rehabilitation plan and interventions.
  6. Recognize and report significant medical observations during assessment and treatment of patients.
  7. Complete thorough written assessments for clients requiring in-person assessments.
  8. Collaborate with the business development team, admissions and discharge coordinator, to share findings and support effective communication with referrers regarding outcomes of clinical triage and safe client admission.
  9. Maintain an effective communication line with HOCS to discuss complex clients or assessments, ensuring decisions are not made in isolation and appropriate clinical guidance is obtained.
  10. Rotate into clinical teams as needed to support client rehab and/or care, maintaining clinical skills.
  11. Hand over client information to relevant QEF CRC staff, including recommendations for therapy input and rehab programs.
  12. Demonstrate the importance of gaining patient and family consent for all interactions and assessments.
  13. Effectively communicate with patients, family members, carers, and colleagues using tact and persuasive skills.
  14. Work in line with QEF policies and procedures.
  15. Contribute to risk assessments and health and safety evaluations for new client admissions.
  16. Participate in the development of the QEF CRC service and support organizational aims and objectives regarding models of care and clinical pathways.
  17. Work effectively within the service, liaising with members of the MDT.
  18. Maintain accurate and timely written records, complying with QEF policy and HCPC guidance.
  19. Participate in the ongoing development of the role and support the development of protocols and processes in the Triage.
  20. Support the HOCS, General Manager, and therapy team leads with day-to-day planning and delivery of a high-quality service.
  21. Participate in annual audit plans and staff surveys.
  22. Engage in evidence-based practice to further own clinical practice and maintain competency.
Qualification, Skills, and Experience
  1. 5+ years post-qualification clinical experience.
  2. 3+ years community experience.
  3. 3+ years experience working with neurologically impaired adults.
  4. Solid experience working in a team/MDT environment.
  5. Experience working in a rehab setting.
  6. Experience working autonomously.
  7. Experience carrying a caseload.
  8. Experience making decisions based on clinical rationale.
  9. Registered nurse or postgraduate physiotherapist/occupational therapist.
  10. Current NMC or HCPC registration.
  11. Completed Mentorship and/or clinical supervision training.
  12. Evidence of ongoing professional development.
