About the Role

The Senior Transformation Manager will lead the delivery and implementation of service redesign and transformation programmes for all age Learning Disability and Autism (LDA). This will include project planning, implementation, and evaluation.

Key Responsibilities
  • Lead the development and implementation of LDA transformation programmes, ensuring alignment with the South West London Integrated Care Board's (ICB) Joint Forward Plan and the NHS Long Term Plan.
  • Oversee and report on LDA Key Performance Indicators (KPIs), including Annual Health Checks, and develop improvement plans where necessary.
  • Work with Place-based team members, health and care providers, and partners to ensure LDA services are prioritized and focused within local Health and Care Plans.
  • Develop strong relationships with partners at Place, including health and care providers, the local authority, and patients and the public.
  • Chair, facilitate, or contribute to multi-agency meetings with senior system partners, including the chairing of Care, Education, and Treatment Reviews (CETRs) and Dynamic Support Register (DSR) meetings.
  • Lead the maintenance and oversight of local Dynamic Support Registers (DSRs), ensuring effective collaboration between health organizations and local authority partners.
  • Support individualized commissioning and ensure the capacity in the South West London CYP Key Worker service is fully utilized.
  • Work with the SWL Head of Learning Disabilities and Autism to ensure the business support model for the team operates effectively.
  • Take the SWL lead for a programme area, ensuring the ambition for the programme across South West London is met.
Person Specification

The ideal candidate will have:

  • Education to masters level or equivalent level of experience in a specialist area.
  • Extensive knowledge of specialist areas, acquired through postgraduate diploma or equivalent experience or training, plus further specialist knowledge or experience to master's level equivalent.
  • Evidence of continued professional development.
  • Ability to analyze complex issues, understand complex financial issues, and identify key points and information.
  • Comprehensive experience of project principles, techniques, and tools.
  • Ability to prepare and produce concise communications for dissemination to a broad range of stakeholders.
  • Demonstrated capability to plan over short, medium, and long-term timeframes and adjust plans and resource requirements accordingly.
  • Ability to make decisions autonomously, working to tight and often changing timescales.
  • Proven ability to influence and secure improvement and change across multiple organizations.
