Community Health Worker

We are seeking a dedicated and compassionate Healthcare Assistant to join our team at Kent Community Health NHS Foundation Trust. As a key member of our community nursing team, you will play a vital role in providing high-quality care to our patients in their own homes.
Main DutiesAs a Healthcare Assistant, you will be responsible for:
- Providing care to patients in their own homes, as part of a care plan agreed with local nursing teams.
- Working as part of a team to provide care to local patients and families.
- Monitoring the quality of care received by patients.
- Providing feedback to managers on the quality of care.
You will work with patients of all ages, but often with elderly patients, those who have been recently discharged from hospital, those who are terminally ill, or those with physical disabilities. As a Healthcare Assistant, you will work as part of a team to keep people safe and well at home.
RequirementsTo be successful in this role, you will need:
- A relevant level 3 qualification (NVQ/ QCF Diploma or certificate of care / Advanced apprenticeship) or equivalent or working towards.
- A minimum of 2 years' experience as a Healthcare Assistant/ Worker in a health or social care setting.
- Experience of working as part of a team and using Microsoft Office and a variety of IT packages (EPay / E-roster/ AT-P/ Rio).
