Community IV Pharmacist for Medicines Optimisation

A vacancy exists within the Manchester University NHS Foundation Trust for a dedicated and proactive pharmacist to enhance community services as part of the Manchester Local Care Organisation (LCO).
Role Responsibilities:
As a key member of the Community Medicines Optimisation Service (CMOS), you will:
- Deliver a high-quality, comprehensive clinical medicines optimisation service within the LCO.
- Provide expert clinical pharmacy knowledge and guidance on medication management to LCO staff and partner organizations, focusing on cost-effective prescribing and clinical treatment guidelines.
- Support LCO Bed Based Units and other CMOS services with clinical pharmacist expertise.
- Collaborate with multidisciplinary teams to enhance patient care coordination, reduce hospital readmissions, improve medication safety, and ensure compliance.
- Oversee the work of medicines optimisation technicians within the clinical service.
- Engage with GP practices to promote best practices in medication management.
- Implement policies and procedures for safe and cost-effective medication use, aligning with local and national priorities.
- Contribute to the development of new services and medicines optimisation initiatives.
- Assist in delivering a medicines optimisation service across various CMOS work-streams as needed.
Qualifications:
The ideal candidate will be registered with the GPhC as a pharmacist, possess a postgraduate clinical diploma or equivalent (or be in the process of obtaining this qualification), and have relevant experience in hospital or primary care settings.
