Clinical Pharmacist

We are seeking a highly skilled Clinical Pharmacist to join our team at Gtd Healthcare, a leading provider of healthcare services in North West England. As a Clinical Pharmacist, you will play a crucial role in delivering high-quality patient care and contributing to the development of our medicines optimisation services.
Key Responsibilities- Medicines Optimisation: Provide expert advice and support to healthcare professionals on medicines optimisation, ensuring that patients receive the best possible care.
- Prescribing Support: Offer guidance and support to prescribers on prescribing-related matters, including medication queries and repeat prescriptions.
- Medication Reviews: Conduct comprehensive medication reviews to identify opportunities for improvement and optimise patient care.
- Collaboration and Communication: Work closely with healthcare professionals, including GPs, nurses, and other pharmacists, to ensure seamless communication and collaboration.
- Leadership and Development: Contribute to the development of our medicines optimisation services and lead on projects to improve patient care and outcomes.
- Registration: Mandatory registration with the General Pharmaceutical Council.
- Qualifications: Masters degree in pharmacy (MPharm) or equivalent, with specialist knowledge acquired through post-graduate diploma level or equivalent training/experience.
- Experience: Experience of working within primary care and/or general practice as a clinical pharmacist, with an understanding of primary care prescribing, concepts of rational prescribing, and strategies for improving prescribing.
