About the Role

We are seeking a highly motivated and experienced Clinical Pharmacist to join our team at The Pharmacist Network. As a Clinical Pharmacist, you will work as part of a multi-disciplinary team in a patient-facing role, working with colleagues to help resolve medication-related issues, helping to utilise skill mix, improve patient outcomes and ensure better access to healthcare.

Key Responsibilities
  • Support practice staff with prescription and medication queries.
  • Help support the repeat prescription system, deal with acute prescription requests, and medicines reconciliation, providing expertise in clinical medicines advice while addressing both public and social care needs of patients.
  • Support with prescribing practices across the PCN, providing clinical leadership on medicines optimisation and quality improvement.
Requirements
  • Experience using clinical systems Emis / System1.
  • Completed CPPE Pathway / currently on CPPE pathway.
