About the Role

We are seeking a highly motivated and clinically focused clinical pharmacist to join our team as an onsite clinical pharmacist in General Practices within the Little Hulton and Walkden area.

As a clinical pharmacist, you will be part of a multi-disciplinary team working with colleagues to help resolve medication-related issues, improve patient outcomes, and ensure better access to healthcare.

You will support practice staff with prescription and medication queries, help support the repeat prescription system, deal with acute prescription requests, and medicines reconciliation, providing expertise in clinical medicines advice while addressing both public and social care needs of patients.

Key Responsibilities

  • Support practice sites with medicines-related tasks using GP systems (EMIS and System 1)
  • Discharge support (meds rec)
  • General med reviews
  • Supporting patient requests for acute and repeat meds
  • Structured Medication reviews
  • Safety audits
