As an Older Adults Mental Health Practitioner, you will be responsible for providing comprehensive person-centred psychosocial assessments, risk assessments, and formulation of presenting mental health needs. You will build therapeutic relationships, develop care plans, and empower individuals to take control of their health.
Key Responsibilities- Provide high-quality care to older adults with functional mental health conditions
- Conduct comprehensive person-centred psychosocial assessments and risk assessments
- Develop care plans and empower individuals to take control of their health
- Build therapeutic relationships and work collaboratively with colleagues and community partners
- Support and motivate individuals to improve their mental health
We are looking for an experienced and qualified Older Adults Mental Health Practitioner who possesses excellent communication and interpersonal skills. You will have a strong understanding of the needs of older adults with functional mental health conditions and be able to work effectively in a community setting.
Essential criteria include:
- Relevant qualification in mental health nursing or occupational therapy
- Post-qualification experience in the relevant field
- Experience of working with older adults with functional mental health diagnoses
- Good verbal and non-verbal communication skills
- Ability to work independently and as part of a team
Desirable criteria include:
- Experience of community working
- Ability to meet the travel requirements of the post
- Good IT skills
