Job Title: Older Adults Mental Health Practitioner

We are seeking a highly skilled and compassionate Older Adults Mental Health Practitioner to join our team at Derbyshire Healthcare NHS Foundation Trust. As a key member of our High Peak Older Adult Community Mental Health Team, you will play a vital role in delivering high-quality care to older adults with functional mental health conditions.

About the Role

This is an exciting opportunity to work in a dynamic and supportive team environment, where you will have the chance to make a real difference in the lives of our patients. As an Older Adults Mental Health Practitioner, you will be responsible for providing comprehensive person-centred psychosocial assessments, risk assessments, and formulation of presenting mental health needs. You will build therapeutic relationships, develop care plans, and empower individuals to take control of their health.

Key Responsibilities
  • Provide high-quality care to older adults with functional mental health conditions
  • Conduct comprehensive person-centred psychosocial assessments and risk assessments
  • Develop and implement care plans to support individuals in achieving their goals
  • Build and maintain relationships with community partners, including statutory organisations, Voluntary, Community and Social Enterprise (VCSE) organisations, and local communities
  • Support and motivate individuals to improve their mental health by helping them develop self-care skills, build resilience, and better utilise community resources
Person Specification

We are looking for an experienced and qualified Older Adults Mental Health Practitioner who possesses the following skills and qualities:

  • Relevant qualification in mental health nursing or occupational therapy
  • Post-qualification experience in working with older adults with functional mental health diagnoses
  • Experience of community working and ability to meet travel requirements
  • Good verbal and non-verbal communication skills
  • Good IT skills
  • Able to work independently and as part of a team
