Job Title: Older Adults Mental Health Practitioner

We are seeking a highly skilled and compassionate Older Adults Mental Health Practitioner to join our team at Derbyshire Healthcare NHS Foundation Trust. As a key member of our High Peak Older Adult Community Mental Health Team (CMHT), you will play a vital role in delivering high-quality care to older adults with functional mental health conditions.

About the Role

As an Older Adults Mental Health Practitioner, you will be responsible for providing comprehensive person-centred psychosocial assessments, risk assessments, and formulation of presenting mental health needs. You will build therapeutic relationships, develop care plans, and empower individuals to take control of their health.

Key Responsibilities
  • Provide high-quality care to older adults with functional mental health conditions
  • Conduct comprehensive person-centred psychosocial assessments and risk assessments
  • Develop and implement care plans to support individuals in achieving their goals
  • Build therapeutic relationships and empower individuals to take control of their health
  • Work collaboratively with colleagues, key partners, and community members to develop services that meet the mental health needs of the community
Requirements

To be successful in this role, you will need to have:

Relevant qualification in mental health nursing or occupational therapyPost-qualification experience in working with older adults with functional mental health diagnosesExperience of community working and ability to travel across the geographical areaGood verbal and non-verbal communication skillsGood IT skillsAbility to work independently and as part of a teamWhat We Offer
