Job Overview

We are seeking a highly skilled and compassionate Mental Health Practitioner to join our team at Derbyshire Healthcare NHS Foundation Trust. As an Older Adults Mental Health Specialist, you will be responsible for providing high-quality care and support to older adults with functional mental health conditions in a community setting.

Main Duties

The successful candidate will have experience of working with older adults and will be able to build therapeutic relationships, develop care plans, and empower individuals to take control of their health. You will work collaboratively with colleagues, key partners, and the local community to develop services that meet the mental health needs of the community.

Key Responsibilities

  • Undertake comprehensive person-centred psychosocial assessments and risk assessments
  • Develop and implement person-centred care plans
  • Build therapeutic relationships and empower individuals to take control of their health
  • Work collaboratively with colleagues, key partners, and the local community to develop services that meet the mental health needs of the community

Person Specification

We are looking for an individual who is passionate about delivering high-quality care and support to older adults. The successful candidate will have:

  • A relevant qualification in mental health nursing or occupational therapy
  • Post-qualification experience in the relevant field
  • Experience of working with older adults with functional mental health diagnoses
  • Good verbal and non-verbal communication skills
  • Good IT skills
