The University Hospitals Sussex NHS Foundation Trust is seeking a highly skilled and motivated Discharge Coordinator to join our dynamic team. As a key member of our Discharge Team, you will play a vital role in ensuring the safe and timely discharge of patients from our acute care settings.
Main Responsibilities- Manage a personal caseload of wards and coordinate complex discharges from the acute care setting.
- Serve as a Trust expert in discharge planning, assisting and supporting the multidisciplinary team (MDT) in driving and coordinating discharges, ensuring timely and effective interventions from all disciplines.
- Assist wards and teams in understanding and implementing the 'Home First' model.
- Support the Transfer of Care Hub (TOCH) by matching patients to appropriate discharge pathways.
- Provide education and training in discharge planning to members of the MDT and other professional groups as required.
- Participate in the development and implementation of Trusted Assessments for care transfers.
- Monitor discharge performance, identifying gaps in service provision that could negatively impact patient length of stay, delayed transfers of care, or overall patient experience.
- Collaborate with multi-agency representatives to reduce patient length of stay, the number of super-stranded patients, and delayed transfers of care, working to achieve discharge-related performance targets.
- Ensure the Trust's compliance with current legislation regarding discharge planning.
- Ensure adherence to the National Service Framework (NSF) for NHS Continuing Healthcare, acting as the Trust's liaison with the Continuing Healthcare (CHC) Team.
Essential Criteria
- Discharge planning experience.
- Band 6 experience at least a year.
Desirable Criteria
- Discharge Coordinator's experience.
Essential Criteria
- Evidence of having championed diversity in previous roles (as appropriate to role).
- Evidence of having undertaken own development to improve understanding of equalities issues.
