We are seeking a skilled and compassionate Social Care Assistant to join our team at Corus Consultancy. As a Social Care Assistant, you will play a vital role in supporting vulnerable adults and informal carers, working within a preventative, enabling, and rehabilitation framework.
Main Responsibilities- Conduct assessments, monitor, and review the social care needs of service users and informal carers, identifying appropriate solutions and making referrals to external agencies as needed.
- Manage a caseload of varying complexity, excluding cases requiring the expertise of a qualified professional, with supervision from a senior practitioner.
- Take responsibility for casework, promoting independence and assessing and managing risk, within an outcome-focused approach and team performance targets.
- Conduct Assistive Technology assessments and arrange provision of suitable equipment.
- Conduct risk assessments and undertake initial enquiries in response to vulnerable adults safeguarding concerns, with support from senior staff, including responding to Merlin Reports.
- Contribute to Continuing Health Care Checklists as part of the Multi-Disciplinary Support Team, using the decision support tool.
- Assess Mental Capacity and act as a Best Interest decision maker when required, in accordance with the Mental Capacity Act, with awareness of Deprivation of Liberty Safeguards.
To be successful in this role, you will need to possess excellent communication and interpersonal skills, with the ability to work effectively with service users, carers, and colleagues. You should also have a strong understanding of the Care Act and Mental Capacity Act, as well as experience in social care or a related field.
