Job Overview

As a Complex Care Worker, you will provide comprehensive support to adults with complex needs, working in a hybrid role that combines office and community-based settings. Your duties will include conducting thorough assessments, developing and implementing person-centred care plans, and collaborating with multiple agencies to ensure integrated support.

Key Responsibilities:

  • Conduct comprehensive assessments of individuals' needs, adhering to the Care Act 2014.
  • Develop and implement person-centred care plans, ensuring statutory compliance under the Care Act 2014.
  • Collaborate with multiple agencies to ensure integrated support, following the Mental Capacity Act 2005 and Care Act 2014.
  • Participate in Court of Protection work, including attending hearings if required, in accordance with the Mental Capacity Act 2005.
  • Identify and address safeguarding concerns, ensuring protection under the Care Act 2014 and Mental Health Act 1983.

Requirements:

  • Relevant social work qualification
  • 3-4 years post-qualification experience, including ASYE and supervision
  • Knowledge of Court of Protection work and attendance when required
