Job Role Overview

As a Complex Care Social Worker, you will be working with adults who require comprehensive support and care. This role involves working closely with individuals, their families, and other healthcare professionals to ensure the best possible outcomes.

Key Responsibilities

Your key responsibilities will include:

  • Assessment and Care Planning - Conduct comprehensive assessments of individuals' needs and develop personalized care plans to ensure their safety and well-being.
  • Collaboration and Coordination - Work closely with other healthcare professionals, including doctors, nurses, and therapists, to ensure seamless care delivery.
  • Safeguarding and Crisis Intervention - Identify and address any safeguarding concerns and develop strategies to prevent crises from occurring.

Requirements and Benefits

To be successful in this role, you will need:

  • Relevant Qualifications and Experience - A social work qualification and at least 3-4 years of post-qualification experience, including supervision experience.
  • Knowledge and Skills - A strong understanding of the Care Act 2014, the Mental Capacity Act 2005, and Court of Protection work.
