About the Role

We are seeking a highly skilled and experienced Specialist Pharmacist to join our team in Frailty and Healthcare for Older People services. As a Specialist Pharmacist, you will work closely with our Healthcare for Older People and Frailty Teams to provide expert pharmacy input and support.

Key Responsibilities
  • Provide specialist pharmacy input to support the development of Frailty and Healthcare for Older People services.
  • Work collaboratively with multidisciplinary teams to deliver high-quality patient care.
  • Develop and implement pharmacy services to improve patient outcomes and experience.
  • Contribute to the development of policies and procedures to ensure safe and effective pharmacy practice.
  • Participate in research and quality improvement initiatives to advance pharmacy practice and patient care.
