Main Responsibilities:
As a Community Care Supervisor, you will play a crucial role in the continuous support of both caregivers and service recipients, ensuring the provision of top-notch care services.
Community Care Supervisors are expected to spend the majority of their working hours in the field executing their primary duties, while also engaging in a certain amount of office-based administrative tasks.
- Accept, allocate, and process new referrals for care and support promptly.
- Process modifications to clients' care and support plans.
- Ensure adequate coverage to deliver care and support punctually.
- Oversee the allocation of caregivers to optimize efficiency while assisting them with our IT systems.
- Collaborate with the office team to maintain accurate electronic and handwritten records.
- Work alongside the Care Team and other health and social care professionals to provide high-quality home care services and implement improvements as necessary.
- Monitor the performance standards of care staff through spot checks, supervisions, and observations.
- Conduct client reviews both on-site and via phone.
- Ensure all documentation in the clients' files is completed and client information is recorded in accordance with company policies and procedures.
- Report to the manager on any aspect of the client's care that, in the opinion of the Care Supervisor, requires investigation or urgent action.
- Assist within the office when necessary and cover for holidays and sickness.
- Participate in On-call duties, which are shared within the office team.
