About the RoleWe are seeking a highly skilled and experienced Mental Health Practitioner to join our team as a Community Psychiatric Nurse. As a key member of our multidisciplinary team, you will play a vital role in delivering high-quality mental health services to adults in the community.
Key Responsibilities- Provide interventions, advice, and support to service users via telephone and face-to-face crisis assessments in various settings.
- Ensure service users receive the most appropriate care and treatment following triage and assessment, referring to the most suitable mental health provision or signposting to relevant services.
- Deliver gatekeeping functions for admission to inpatient mental health units and offer in-reach work in wards to facilitate discharges.
- Plan, implement, and evaluate programs of care for clients in crisis with acute mental health problems in the community, coordinating activities with other professionals and agencies.
- Provide advice, support, and information to service users, carers, GPs, and other professionals requesting information regarding a crisis situation.
- Current and valid registration with the NMC.
- Proven experience within a CMHT or community mental health setting.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a multidisciplinary team.
- Commitment to providing high-quality patient care.
- Members of staff should be a driver and have Crisis Team or Psych Liaison experience.
