About the Role

We are seeking a highly motivated and compassionate Recovery Outreach Worker to join our Mental Health and Complex Needs Service at Hestia Housing Support.

Key Responsibilities
  • Provide holistic, person-centered support to service users with severe and enduring mental health needs, promoting recovery and social inclusion.
  • Develop and implement effective support plans, working closely with service users, their families, and other professionals to achieve their goals and aspirations.
  • Visit service users in their homes or community locations, providing emotional support and guidance to help them maintain their independence and well-being.
  • Work collaboratively with other agencies to ensure seamless support and minimize unplanned hospital admissions.
  • Conduct regular needs and risk assessments, reviewing and updating support plans as necessary.
Requirements
  • Experience of working with individuals requiring support related to mental ill health and complex needs.
  • Ability to deliver support in a community/outreach setting, promoting recovery and social inclusion.
  • Excellent knowledge of mental health issues, including signs, symptoms, and treatments.
  • Knowledge of key working, person-centered support planning systems, and the recovery approach.
  • Understanding of equality and diversity, and the importance of promoting service users' rights, privacy, dignity, and choice.
