Palliative Care Nurse Job Description

Trinity Hospice is seeking a compassionate and skilled Palliative Care Nurse to join our team. As a Palliative Care Nurse, you will provide high-quality care and support to patients and their families, ensuring their physical, emotional, and spiritual needs are met.

Key Responsibilities:
  1. Assess and develop individualized care plans for patients with terminal illnesses.
  2. Provide symptom management and pain control, working collaboratively with the medical team.
  3. Lead the ward round and delegate tasks to the nursing team, ensuring seamless care delivery.
  4. Communicate effectively with patients, families, and healthcare professionals to ensure a patient-centered approach.
  5. Participate in multidisciplinary team meetings to discuss patient care and develop strategies for improvement.
  6. Contribute to the development and implementation of palliative care policies and procedures.
  7. Maintain accurate and comprehensive patient records, ensuring compliance with regulatory requirements.
Requirements:
  • Registered Nurse with a valid nursing license.
  • Experience in palliative care or a related field.
  • Strong communication and interpersonal skills.
  • Ability to work effectively in a team environment.
