Job Title: Associate Hospice Nurse Specialist

This is an exciting opportunity for a skilled and compassionate nurse to join our community palliative care team. As an Associate Hospice Nurse Specialist, you will work closely with patients, carers, and healthcare professionals to provide high-quality, patient-centered care in the community.

Main Responsibilities:
  • Assess patients with palliative care needs and develop individualized care plans to maximize their quality of life.
  • Provide specialist advice and support to patients, carers, and healthcare professionals on palliative care and symptom management.
  • Work collaboratively with the community team to develop and deliver management plans, ensuring seamless care transitions and coordination.
  • Contribute to the development of Single Point of Access and Virtual Ward projects, promoting efficient and effective care delivery.
  • Deliver education and training to healthcare professionals on palliative care, ensuring they have the skills and knowledge to provide high-quality care.
Requirements:
  • First-level RGN with a minimum of 3 years post-registration experience.
  • Registered with the NMC.
  • Advanced communication skills and the ability to work independently and as part of a team.
  • Experience of working in palliative care and community settings.
  • Ability to prioritize workload and manage competing demands on time.
