Job Description

**About the Role**

We are seeking an experienced Advanced Clinical Pharmacist to join our team at Primary Care 24. As a key member of our organization, you will play a crucial role in developing and delivering our medicines optimization and safety agenda.

Key Responsibilities:

  • Develop and implement medicines optimization strategies to improve patient outcomes
  • Contribute to the management of long-term conditions and repeat prescribing
  • Undertake clinical medication reviews and assessments to identify areas for improvement
  • Work with care home staff to improve the safety of medicines ordering and administration
  • Reconcile medicines following discharge from hospitals and intermediate care

Requirements:

  • Degree in Pharmacy
  • Independent Prescriber V300
  • Experience of working in urgent care and/or primary care
  • Minimum of 2 years post-qualification experience
  • In-depth therapeutic and clinical knowledge and understanding of evidence-based healthcare
  • Experience of medication review, face-to-face and remotely
