Job Title: GP Pharmacist - Liverpool

We are seeking an experienced Clinical Pharmacist to join our team in Liverpool. As a key member of our multidisciplinary team, you will provide clinical pharmacy leadership and support to our GP practice.

Key Responsibilities:
  • Provide clinical pharmacy leadership and support to our GP practice
  • Conduct Structured Medication Reviews and chronic disease management
  • Optimise medicines use and improve patient outcomes
  • Support practice staff with prescription and medication queries
  • Provide telemedicine support and medicines information
  • Contribute to quality improvement initiatives and the quality and outcomes framework
Requirements:
  • Current registration with the GPHC as a Pharmacist
  • Previous experience working in a GP practice
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
