Job Title: Clinical Pharmacist

We are seeking an experienced Clinical Pharmacist to join our team in Liverpool. As a Clinical Pharmacist, you will work closely with our multidisciplinary team to provide clinical pharmacy leadership and support to our GP practice.

Key Responsibilities:
  • Provide clinical pharmacy leadership and support to our GP practice
  • Conduct structured medication reviews and chronic disease management
  • Develop and implement quality improvement initiatives
  • Collaborate with practice staff to optimize medication use and improve patient outcomes
  • Provide medicines information and support to practice staff and patients
Requirements:
  • Current registration with the General Pharmaceutical Council (GPhC)
  • Experience working in a GP practice or similar setting
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
