Job Title: Clinical Pharmacist - Liverpool

We are seeking an experienced Clinical Pharmacist to join our team in Liverpool. As a Clinical Pharmacist, you will work as part of a multidisciplinary team to provide clinical pharmacy leadership for the practice, taking responsibility for structured medication reviews, chronic disease management, medicines optimisation, quality improvement, and other enhanced services.

Key Responsibilities:
  • Provide support to practice staff with regards to prescription and medication queries, ensuring integration with community and hospital pharmacy to help utilise skill mix, improve patient outcomes, and manage GP workload.
  • Clinical Medication Reviews
  • Telephone Medicines support
  • Medicines Information to practice staff and patients
  • Unplanned hospital admissions
  • Document management
  • Medicines quality improvement
  • Medicines safety
Requirements:
  • Current and valid registration with the GPHC as a Pharmacist
  • Previous experience working within a GP Practice
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a multidisciplinary team
  • Clinical knowledge, skills, and experience
