Main Responsibilities
- Conduct comprehensive medication reviews and assessments to identify and resolve medication-related problems for patients.
- Collaborate with physicians and other healthcare professionals to develop and implement personalized medication therapy plans.
- Provide patient education on medication use, potential side effects, and drug interactions.
- Monitor and adjust medication therapy based on patient response and laboratory values.
- Conduct medication reconciliation and identify discrepancies to ensure accurate and up-to-date medication lists.
- Participate in multidisciplinary meetings to discuss patient care and optimize treatment outcomes.
- Provide education sessions for GPs, practice staff, other health professionals, and patient groups.
- Stay up-to-date with current literature and guidelines to provide evidence-based pharmaceutical care.
