Hunter Gatherer Mental Health is seeking a skilled and dedicated CPN Care Coordinator to join their Adult team in Greater Manchester. As a key member of the team, you will be responsible for coordinating, commissioning, and delivering individual packages of care for a caseload of service users within Community Mental Health Teams (CMHTs).

Key Responsibilities:
  • Undertake initial assessments of new referrals to the service
  • Support the duty system
  • Coordinate, commission, and deliver individual packages of care for a caseload of service users
Requirements:
  • Current and valid registration with the NMC
  • Proven experience within a CPN recovery role
