Job Title: Care Coordinator

At Pennine Care NHS FT, we are seeking a highly skilled and dedicated Care Coordinator to join our team. As a Care Coordinator, you will play a vital role in providing high-quality care and support to individuals with complex mental health needs.

Key Responsibilities:
  • Assess the needs of service users and develop personalized care plans
  • Coordinate and deliver care packages, including medication management and risk assessment
  • Work collaboratively with multidisciplinary teams to ensure seamless care delivery
  • Provide education and support to service users and their families
  • Monitor and evaluate care plans, making adjustments as needed
Requirements:
  • Registered Mental Health Nurse, Occupational Therapist, or Social Worker
  • Experience working with individuals with complex mental health needs
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Basic IT skills and proficiency in electronic record-keeping
