About the Role

We are seeking a highly skilled and experienced mental health professional to join our Home Treatment Team as a Senior Mental Health Practitioner. As a key member of our multidisciplinary team, you will play a vital role in delivering high-quality care to individuals experiencing mental health crises in their own homes.

Key Responsibilities
  • Develop and implement individualized care plans for service users, incorporating signposting and support as needed.
  • Provide intensive home treatment and crisis management interventions in community settings.
  • Collaborate with other professionals and agencies to ensure seamless care coordination and effective service delivery.
  • Evaluate and record outcomes of mental health assessments, adapting care plans accordingly.
  • Involve family and carers in assessment and care planning, with service user consent.
  • Provide education and self-help resources to patients and carers, tailored to individual needs.
Requirements
  • Registered Mental Health Nurse, Occupational Therapist, or Social Worker with extensive experience in mental health.
  • Knowledge of mental health legislation and guidance, with experience working within a multidisciplinary team.
  • Ability to provide leadership cover and supervise community support workers.
  • Excellent communication and interpersonal skills, with a commitment to professional development.
  • Flexibility to work across a 7-day week, responding to client needs.
