About the Role

We are seeking a highly skilled and experienced mental health professional to join our Home Treatment Team as a Senior Practitioner. The successful candidate will be responsible for delivering high-quality, recovery-focused interventions to adults in mental health crisis, working in close collaboration with our multidisciplinary team.

Key Responsibilities
  • Develop and implement individualized care plans for service users, incorporating risk management and crisis intervention strategies.
  • Provide intensive home treatment and short-term psychotherapeutic interventions as needed.
  • Liaise with other healthcare professionals and agencies to ensure seamless care coordination and effective service delivery.
  • Conduct thorough risk assessments and develop strategies to mitigate risks, ensuring the safety and well-being of service users.
  • Work collaboratively with the multidisciplinary team to develop and implement treatment plans, ensuring a comprehensive and coordinated approach to care.
Requirements
  • Qualified Mental Health Nurse, Occupational Therapist, or Social Worker with significant post-qualification experience in mental health.
  • Proven ability to work autonomously in the community, with excellent assessment and risk management skills.
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
  • Commitment to ongoing professional development and staying up-to-date with best practices in mental health care.
