Job Summary

The Early Intervention Team at Pennine Care NHS FT is seeking a highly skilled and dedicated Care Coordinator to join our multidisciplinary team. As a Care Coordinator, you will play a vital role in supporting individuals experiencing a First Episode of Psychosis, providing comprehensive support and intervention to help them achieve their recovery goals.

Main Responsibilities
  • Assess and manage complex caseloads, delivering high-quality support and interventions in line with best practice and NICE guidelines.
  • Work collaboratively with the multidisciplinary team to develop and implement individualized care plans, ensuring seamless coordination of care.
  • Provide clinical supervision and guidance to other mental health professionals, promoting excellence in practice and service development.
  • Contribute to the development of the service, promoting the Early Intervention Team's work to other agencies and stakeholders.

Requirements
  • Registered Mental Health Nurse (RMN), Occupational Therapist, or Social Work registration.
  • Postgraduate qualification or equivalent experience relevant to the role.
  • Experience of working with clients with serious mental illness and their families/carers.
  • Experience of care coordinating complex packages of care.
  • Experience of conducting high-quality risk assessments and implementing risk management plans.
Desirable Qualifications and Experience
  • Approved Mental Health Practitioner.
  • Mentorship/Practice Leadership qualification.
  • Experience in early intervention for psychosis, specialist placement, or previous employment in the early intervention service.
  • Experience in the use of psychosis-specific semi-structured interviews (PANSS; CAARMS; PSYRATS).
Working Arrangements

This is a full-time position, and the successful candidate will be required to work across the Trust footprint, with access to remote working systems. A car or means of mobility is essential for this role.
