About the Role

We are seeking a dedicated and experienced Older Adult Community Mental Health Nurse/Practitioner to join our team at Dorset Clinical Commissioning Group. In this role, you will play a vital part in providing high-quality care and support to older adults within their communities.

Key Responsibilities

  • Conduct comprehensive assessments of older adults' mental health needs.
  • Develop and implement individualized treatment plans, utilizing evidence-based therapeutic interventions.
  • Collaborate with multidisciplinary teams to ensure holistic care for patients.
  • Provide education and support to patients, families, and caregivers.
  • Contribute to the management of change within the service.

Essential Criteria

  • Possess a degree or diploma in nursing with a relevant professional qualification, or equivalent experience.
  • Demonstrate a strong understanding of assessment and therapeutic interventions for older adults.
  • Have experience in contributing to the management of change within a healthcare setting.
  • Be able to effectively communicate with patients, families, colleagues, and other stakeholders.
  • Exhibit strong analytical and problem-solving skills.

Desirable Criteria

  • Recent experience in a comparable role within community mental health.
  • Experience in providing clinical supervision and mentoring to junior staff.
