We are seeking a highly skilled and experienced Mental Health Nurse to join our team at Pennine Care NHS Foundation Trust. As a Clinical Lead for Community Mental Health, you will be responsible for providing comprehensive mental health services to older adults with specialist needs.
Key Responsibilities- Develop and Implement Care Plans: Collaborate with multidisciplinary teams to create and implement personalized care plans for clients, ensuring their needs are met and their quality of life is improved.
- Lead Clinical Supervision: Provide guidance and support to junior staff, promoting their professional development and ensuring they have the skills and knowledge to deliver high-quality care.
- Collaborate with Stakeholders: Work closely with primary health care teams, social services, and other agencies to ensure comprehensive care for clients, promoting an integrated service.
- Monitor and Evaluate Care: Regularly review and assess the effectiveness of care plans, making adjustments as needed to ensure the best possible outcomes for clients.
- Participate in Mandatory Training: Engage in ongoing professional development, staying up-to-date with the latest research, guidelines, and best practices in mental health care.
- Education and Qualifications: Registered Mental Health Nurse (RMN) with postgraduate experience and short courses to postgraduate diploma level.
- Experience: Minimum of four years post-qualification experience in mental health, with experience of acting as a care coordinator under the Care Programme Approach.
- Knowledge and Skills: Ability to provide leadership cover, knowledge of mental health legislation and guidance, and sound communication skills.
