About the Position
Humankind is honored to be a key provider within the Reconnected to Health initiative, which delivers comprehensive drug and alcohol recovery services across all seven North East Prison establishments. This initiative is led by Spectrum Community Health CIC and includes collaboration with Tees, Esk and Wear Valleys NHS Trust and Humankind to ensure holistic and integrated care for individuals in custody throughout the region. Our services include drug and alcohol recovery coordination, psychosocial interventions, and continuity of care planning within the prison healthcare framework. We maintain strong partnerships with community services nationwide, ensuring seamless transitions for our clients.

The Reconnected to Health program is designed to assist residents in secure environments in overcoming drug and/or alcohol dependencies.

The primary objective of the Healthcare Navigation Reconnect Service is to eliminate health-related obstacles that hinder individuals' engagement both prior to and after their release from prison. We aim to reduce health inequalities and enhance the wellbeing of those at low to medium risk of reoffending. As a Reconnect Navigator, your role will involve promoting ongoing care support for individuals transitioning out of prison, fostering positive and sustainable connections within their local communities, and building recovery capital assets.

The salary mentioned reflects the full earning potential for this role, with starting salaries allowing for pay progression within the designated pay band.

Key Requirements
The use of a motor vehicle and possession of a full driving license are essential for this role.
