Job Overview

Are you seeking flexible and additional working sessions that accommodate your schedule? This is an excellent opportunity for a qualified Advanced Nurse Practitioner (ANP) with independent prescribing capabilities. This role allows you to enhance the efficiency of patient care by evaluating, planning, administering treatment, and either discharging or referring patients to other healthcare professionals or agencies, all through our primary care service. You will also engage with our electronic prescribing system. As part of our North West London Out of Hours (OOH) service, you will operate independently within your professional competency alongside a diverse team, including advanced nurse practitioners, registered nurses, urgent care practitioners, mental health and dental nurses, as well as prescribing pharmacists and pharmacy advisors. We provide discounted national updates and clinical topic refreshers, along with numerous regional and national career development opportunities. This role is centered around our primary care service for in-person consultations.

Key Responsibilities

Ensure seamless patient pathways across primary, community, secondary, and tertiary care. Adopt a patient-centered approach to service delivery, grounded in a solid understanding of various service delivery models. Be accountable for assessing the health and/or social needs of patients within the primary care setting, utilizing established clinical protocols and guidelines. Integrate health education into the nursing role within the unscheduled care environment. Continuously enhance clinical competence and knowledge of care delivery within the primary care setting through formal and informal training. Collaborate closely with medical practitioners to provide high-quality, evidence-based care to patients in the primary care setting or their homes. Specific Duties for Advanced Nurse Practitioners Manage a virtual caseload, ensuring the delivery of integrated, quality patient care while working autonomously. Take a leading role in providing palliative care and other specialized nursing services within the unscheduled care environment. Deliver holistic patient care within the unscheduled care setting, from initial assessment through to diagnosis and treatment, including the prescription of medications in accordance with established clinical guidelines. Hold a nurse prescribing certificate and demonstrate competence in prescribing for patients while working independently.

About Us

What you will need: Recent experience as an advanced nurse practitioner in a primary or urgent care setting, with a proven track record of assessing, treating, and discharging patients of all ages with unscheduled presentations. Qualifications in patient assessment, independent prescribing, and a valid NMC PIN are essential. Strong listening and communication skills, along with proficient IT and keyboard skills, are required. You should also be current with all compliance documentation, including mandatory training. We offer discounted national update/clinical topic refreshers and a host of regional and national career development opportunities. This role is based in our primary care service for face-to-face sessions.
