Position Overview

We are pleased to announce a rewarding opportunity within our Acute Frailty service. We are seeking a part-time qualified Senior Clinical Practitioner with expertise in Frailty. The successful candidate will collaborate within our Consultant-led multidisciplinary team in the specialized Frailty SDEC unit. As a pivotal leader in the provision of acute frailty services across the region, this unit facilitates direct community and emergency service referrals while enhancing patient flow through the emergency department.

Key Responsibilities

This role presents a unique opportunity to contribute to our newly established multidisciplinary Frailty Assessment Unit (FAU). This innovative unit offers an alternative pathway for older adults experiencing frailty who require emergency care. It is strategically located alongside our Emergency Department and accepts direct referrals from community services and emergency responders.

Candidate Profile

Essential Qualifications

  • Professional registration with NMC / HCPC / GPC
  • Prescribing Qualification or eligibility to undertake within six months of appointment
  • Management and Leadership qualification
  • Extensive demonstrable experience in Frailty
  • Proven experience in implementing evidence-based practices
  • Exceptional clinical and practical skills as an autonomous practitioner
  • Strong communication abilities with staff at all levels
  • Capacity to recognize and analyze complex situations and take appropriate action
  • High level of personal motivation

Desirable Qualifications

  • Proficiency in information technology
  • Experience in research activities
  • Background in Service Improvement / Quality Improvement / Practice Development
  • Basic understanding of financial management
