Youth Transition Advisor for Care Leavers

Belmont Recruitment is seeking a dedicated Support Advisor for Young Care Leavers to join our team, working with a local council on a part-time basis of 16 hours per week.
The primary objective of this role is to provide guidance, support, and resources to empower young individuals in maximizing their educational, vocational, and housing opportunities.
Key Responsibilities:
- Ensure that young individuals receive appropriate support at crucial times to facilitate their transition into adulthood and independence from care.
- Participate in the evaluation of care leavers and their families who require assistance, providing support under the guidance of a senior practitioner or team leader.
- Collaborate in the creation and execution of Pathway Plans for young individuals in care, aged 15 years and older, in consultation with the young person and relevant stakeholders. Lead the preparation of these plans alongside the assigned social worker.
- Assist social workers and other professionals in implementing assessments, interventions, and pathway plans, contributing to comprehensive evaluations of service user needs.
- Encourage active participation of care leavers in their leaving care plans and oversee the monitoring and evaluation of these plans.
- Engage with care leavers in service development and empower them to build their skills and confidence to advocate in various settings.
- Establish networks and partnerships to enhance and develop services for care leavers.
- Adhere to the relevant legislation and departmental guidelines regarding leaving care, ensuring compliance with administrative procedures.
- Maintain accurate records of young individuals and update information systems as necessary. Assist in the preparation of reports as required.
- Provide advice to social workers on leaving care matters and adapt to the evolving needs of children and young people.
