About the Role

We are seeking a compassionate and skilled Healthcare Assistant to join our multidisciplinary community-based team in Derbyshire and South Staffordshire. As a key member of our team, you will provide holistic, person-centred care to patients and their loved ones, supporting them through the end-of-life journey.

Key Responsibilities
  • Deliver high-quality personal care and support to enhance wellbeing, prioritizing the needs of each individual.
  • Hold person-centred conversations, practicing empathetic communication with patients and their loved ones in a supportive and professional manner.
  • Develop and evaluate care management plans alongside other healthcare professionals, ensuring the complex needs of each person are met.
  • Maintain regulatory registration requirements and adhere to national professional and occupational standards associated with the role.

Requirements
  • National Vocational Qualification (NVQ) Level 3 in Health and Social Care or an acceptable alternative qualification.
  • A minimum of six months of recent care delivery experience in a Health or Social Care Environment, providing care and caring for dying patients.
  • Proven ability to develop and evaluate care management plans, prioritizing the needs of each individual.
  • Thorough understanding of the physical, psychological, social, and spiritual domains of palliative care.
