As a key member of our multi-disciplinary team, you will play a vital role in delivering high-quality care to adults with moderate to severe mental health difficulties. In this Lead Practitioner position, you will coordinate care by working closely with individuals who are struggling with various mental health conditions, providing them with the support and guidance they need to recover and improve their overall quality of life.

Key Responsibilities
  • Assess patients and develop personalized care plans to meet their unique needs
  • Provide expert care and support to individuals in their own homes or in the community
  • Develop and implement risk assessments, collaborative care planning, and medication management
  • Monitor patients' physical health and well-being
  • Manage patient documents and perform other general nursing duties

Requirements
  • Registered Mental Health Nurse Degree or equivalent
  • Evidence of commitment to continuing professional development
  • Understanding of Health and Safety policies and procedures
  • Knowledge and practice of Mental Health Act 1983
  • Experience of assessing, planning, implementing, and reviewing care needs
  • Experience of CPA Care Coordinator role
  • Experience of care planning, risk assessing, and safety planning
