Community Staff Nurse

About the Role:
We are seeking a highly skilled and dedicated Community Staff Nurse to join our team at First Community Health and Care. As a Community Staff Nurse, you will play a vital role in providing high-quality nursing care to patients in their own homes or residential settings.
Key Responsibilities:
- Assess, plan, implement, and evaluate individualized care under the direction of a Clinical Team Lead and District Nurse.
- Work closely with health and social care professionals across the primary care networks to prevent patients from being admitted to hospital.
- Provide nursing care for a wide range of conditions, including assessing, planning, implementing, and evaluating individualized care.
- Participate in the admission and discharging planning of patients from the caseload.
- Promote the wellbeing, health promotion, and self-management of patients' health needs.
- Work within First Community and local policies, guidelines, procedures, and SOPs.
- Supervise and delegate care provided by more junior staff, while maintaining accountability.
- Maintain records in accordance with First Community's policy and NMC Guidelines, including electronic patient and data management systems (EMIS).
Requirements:
- Registered Nurse (RN1) qualification.
- Practice Supervisor (PS) qualification or willingness to complete PS development.
- Excellent verbal and written communication skills.
- IT skills.
- Flexible, lateral thinker.
- Reflective practitioner.
- Proven ability to motivate others.
- Able to work competently without supervision.
- Responsible for own professional development.
- Car Driver with a valid license to drive in the UK and business use insurance.
