We are seeking a skilled and compassionate Community Nurse to join our team at First Community Health and Care. As a Community Nurse, you will play a vital role in providing high-quality nursing care to adults and their families in their own homes or residential settings.
Key Responsibilities
- Assess, plan, implement, and evaluate individualized care under the direction of a Clinical Team Lead and District Nurse.
- Work closely with health and social care professionals across the primary care networks to prevent patients from being admitted to hospital.
- Provide nursing support to adults and their families, promoting a healthy lifestyle and independence where possible.
Requirements
- Registered Nurse (RN1) qualification.
- Practice Supervisor (PS) qualification or willingness to complete PS development.
- Excellent verbal and written communication skills.
- Ability to work independently and as part of a team.
