We are seeking a skilled and dedicated Community Health Nurse to join our Intermediate Care Team at First Community Health And Care. As a Community Health Nurse, you will play a vital role in assessing the health and social care needs of patients and their carers within the community, aiming to prevent hospital admissions and facilitate discharge home.
Main Responsibilities
- Assess the health and social care needs of patients and their carers within the community
- Develop and implement individualized care plans to promote independence and rehabilitation
- Work closely with the multidisciplinary team to ensure seamless care delivery
- Act as a role model for junior team members and contribute to the promotion of an environment that encourages and supports development of staff using evidence-based nursing practice
Requirements
- Relevant NMC qualification
- Minimum of 2 years post graduate experience
- Experience of initiating and participating in clinical audit
- Excellent communication and interpersonal skills
- Ability to organise the work load, able to delegate and prioritise
