Job Overview
This role is an exciting opportunity to join the NELFT NHS Foundation Trust as a Specialist Community Public Health Nurse - Health Visitor. As a key member of our team, you will be responsible for delivering high-quality healthcare services to children and families in our community.
Main Responsibilities
- Lead the delivery of the Healthy Child Programme, ensuring the service model is implemented in practice.
- Manage caseload allocation and classification of cases in line with the national strategy of Universal, Universal plus, and Universal partnership plus.
- Work in accordance with the Locality commissioned service specification, ensuring the 5 mandated reviews are completed within required timescales and services provided are evidence-based practice.
- Improve the health and wellbeing of children and young people, especially those from disadvantaged groups.
Requirements
- Specialist Community Public Health Nurse/Health Visitor qualification.
- Current registration with the Nursing and Midwifery Council (NMC).
- Experience of the Healthy Child Programme.
- Partnership working with external agencies and organisations.
