About the RoleThis is an exciting opportunity to lead a team of specialist community public health nurses, providing expert care and support to children and families in our community. As a District Nurse Team Leader, you will be responsible for managing a caseload of patients, working closely with other healthcare professionals, and contributing to the development of our services.
Key Responsibilities- Lead the delivery of the Healthy Child Programme, ensuring the service model is implemented in practice.
- Manage caseload allocation, classification of cases, and ensure services are provided within the 4-tier model.
- Work in accordance with the Locality commissioned service specification, ensuring mandated reviews are completed within required timescales.
- Improve the health and wellbeing of children and young people, especially those from disadvantaged groups.
- Specialist Community Public Health Nurse/Health Visitor qualification.
- Current registration with the Nursing and Midwifery Council (NMC).
- Experience of the Healthy Child Programme.
- Ability to demonstrate innovation and motivation.
