About the Role

Proud to Care Devon is seeking a skilled Adult Care Specialist to join our Exeter South Community Health & Social Care Team. As a key member of our team, you will play a vital role in assessing the care needs of adults with physical disabilities and identifying suitable outcomes.

Key Responsibilities
  • Conduct comprehensive assessments of adult care needs, taking into account individual strengths and circumstances.
  • Develop and implement personalized care plans, working closely with service users, families, and healthcare professionals.
  • Collaborate with local providers to ensure seamless delivery of care services, promoting a progressive working relationship.
  • Work with adults with a range of needs, including those with dementia, Parkinson's disease, Huntington's, and Motor Neuron Disease.
  • Participate in Education & Health Care Plan (EHCP) reviews, ensuring the best possible outcomes for service users.
Requirements
  • Robust knowledge of the Care Act 2014 and other relevant legislation, including the Mental Capacity Act and Safeguarding Adults.
  • Ability to work independently and as part of a multidisciplinary team, promoting a culture of collaboration and respect.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with service users, families, and colleagues.
  • Confidence and fluency in English, with the ability to communicate complex information effectively.
  • DBS disclosure and a commitment to safeguarding and promoting the welfare of vulnerable adults.
Working Arrangements
